The expandable insulated lunch box includes a freezer gel pack which could leak poisonous chemicals if ingested.
Recently, a dog reportedly died after chewing on the gel pack.
To know if your lunch box is on this recall list, look for code set "1-61731-99-57," printed on one of the two white labels sewn under the white fastener inside the main compartment.
Consumers can return the lunch box set to Costco for a full refund or may receive a $5 cash refund for the gel pack only by calling 800-722-2545 for information.
